I picked up a set that works similar to it at Lowes. Mine is made by GearWrench, so you should be able to get it any place that sells GearWrench (including Sears).

They are a set of of adapter "sockets" that you insert into the wrench (hopefully a GearWrench ratcheting box end wrench), and then attach a socket to the adapter. Then you use the wrench just like a ratchet (the knurled top of the adapter allows you to spin it with your fingers as necessary). It makes for a very low profile - although not quite as low as that shown in the photos.
