I would say you do not have to put the alum angled spacer in there for the cable bracket, but it is made to make the cables at a better angle and should have smoohter operation for it. If you can put it in you should.

On the notchy feeling. I would try to make sure you adjust the rod end at the console side for the crossgate lever. Try another turn in and it my solve the problem. If it feels notchy going into 5th gear that will help and make it feel good and smooth.
